Initial Osteopathic Evaluation & Treatment
70-85 min visit with hands-on diagnostic evaluation and treatment.
Includes comprehensive discussion of medical history and sometimes additional clinical examination.​

Home visits
May be available on a case-by-case basis for established patients in Maryland. Prices start at $500 and are based on the time required for the visit plus travel.

Virtual visits
Pricing is the same as in-person at this time. Virtual visits can be helpful for education, discussion, obtaining part of an extended history, or walking a family member through a simple home treatment.

Paperwork, Etc.
If forms, reports, or records are requested and require time to complete outside of your visit, you will be accordingly charged for time spent and any costs of supplies and postage. Paperwork sent to the office by insurance companies will not be replied to. Dr. Levy will not participate in trials, testify, or do depositions for patients.
Payment
We are a fee-for-service practice which means that payment is due in full at the time of visit.

We accept cash, check (or Zelle), money order, HSA/FSA cards, credit cards (Visa, MasterCard, American Express, Discover), and debit cards (Visa and Mastercard). Gift cards are also available.

Do you take insurance?
Unfortunately, we are not able to accept insurance or other third-party payments as the time and resources required detract from our ability to spend the time we need with our patients. You will be given a superbill with all the codes needed for you to submit to your insurance company, including diagnosis codes, visit codes, and OMT procedure codes. If you can go out of network, we have been told that insurance companies have covered between 20%-90% of the bill, and the visit costs may go toward your out of pocket or deductible. If your insurance requires a referral to see a specialist, you may need a referral to be reimbursed (ask your PCP for a referral for Osteopathic Manipulative Medicine).

Medicare (including Medigap), Medicaid, and some specific insurances such as Kaiser Permanente will NOT provide reimbursement. Our practice has opted out of Medicare and therefore claims should not be submitted for reimbursement to Medicare. All patients with Medicare will be asked to sign a Medicare Opt-Out form which acknowledges that you understand that Dr. Levy has opted out and does not accept Medicare payment. If you have a secondary insurance, you can submit the superbill and a copy of your signed Medicare Opt-Out form to the secondary insurance, and they may be able to reimburse you for at least a portion of our fees, depending on coverage.

If you have any questions about reimbursement or requirements for reimbursement, please ask your insurance company first! We do not know how or IF your specific insurance plan will reimburse. If your insurance company requires our office to fill out forms for you to get reimbursed, please note that we cannot do this as it requires administrative resources beyond our capacity. Paperwork from insurance companies will not be responded to if sent to the office. If your insurance company requires an office visit note or something simple that you can provide to them directly, please let us know at your visit so we can try and help you if possible.
Specific questions to ask your insurance company
If you would like to check with your insurance company about coverage prior to scheduling, you can ask them:

Do they cover out-of-network physician office visits?​

If you have Medicare and are submitting to a secondary insurance, you may want to ask more specifically if they cover visits with out-of-network physicians who have opted out of Medicare.

If you want to ask how much is covered, you can give an example of a CPT code for an office visit is 99205 (typical new adult), 99204 (typical new child), 99214 (typical adult follow up), or 99213 (typical child follow up).

Do they cover osteopathic manipulative treatment or OMT?

Will they cover an office visit ("E&M code") + OMT on the same day?

Some insurances such as Cigna, United, and BCBS have been denying this combination.

Cancellations and Missed Appointments
We only see a few people each day in order to provide as much time as is necessary to give you true quality visits. When someone cancels with short notice, it is often difficult to fill that time slot that was set aside for specifically for that person. When someone misses their appointment, there is no opportunity to give that time to someone else.
As such, a missed appointment without prior notification will result in full charge of the appointment fee.
If we are unable to fill the time slot, a late cancellation (within 48 hours of the appointment) will result in a charge of half the appointment fee.
Future appointments will not be made until the late cancellation or no show charge has been paid in full.

If you have extenuating circumstances, consideration will be given. However, all late cancellations and no shows are noted in the patient record, and patients with excessive late cancellations may be asked to find another office for their care. In case of illness, urgent situations, or unforeseen circumstances, we ask that you notify us as soon as possible so that the time may be used to help another patient.

Patients who miss their first appointment without giving reasonable notice ahead of time will be asked to find care elsewhere.

Inclement Weather/Emergencies
If your appointment needs to be rescheduled due to physician illness or emergency, unforeseen office issues, or due to inclement weather, you will be notified as soon as possible by email or phone. In case of inclement weather, we will try to follow government and school late opening and cancellation procedures.
